How Do I Use the File of L.I.F.E.?

Procedure

• Please complete a File of L.I.F.E. form. Be sure to include the date that you completed this form.
• Attach a recent photograph of yourself to the back page of the File of L.I.F.E. form and put it inside the File of L.I.F.E. pouch.
• Place the File of L.I.F.E. pouch on the outside of your refrigerator door.
